NJ Groups with 2-50 employees must meet the following reform guidelines to purchase a health plan:
- A minimum of 2 eligible employees who work 25hrs/wk or more.
- Employer must contribute a minimum of 10% of the employee monthly premium.
- 75% of employee participation is required.

If your NJ group meets the above requirements, please fill out a CBA Census form to obtain group health quotes.
Groups over 50 lives do not follow NJ reform rules. We would need the following information emailed, faxed, or mailed to us to complete your quote request:
- Full Census including zip codes
- Current & renewal rates (if applicable)
- Current list bill
- Number of years with current carrier
- Employer contribution (employee/dependent)
- Claims experience (if applicable)
- Nature of business
- Plan design
